Breast and Cervical Cancer Care Coordination and Navigation
A grant from The California Endowment for the statewide CCAN Program that served low-income, un/underinsured breast and cervical cancer patients ended in June 2008. However, a program that offers navigation, care coordination and financial assistance for breast cancer patients continues in a 17-county Sacramento Valley region.
Breast Cancer Patient Navigation & Support Project
April 1, 2008 – March 31, 2009
The Patient Navigation and Support Project is operated through the California Health Collaborative, with funding from Susan G. Komen for the Cure Sacramento Valley Affiliate. Participation in the Project is available to uninsured, low-income symptomatic and diagnosed breast cancer patients who reside within the *17-county Sacramento Valley Affiliate service area. The Patient Navigation and Support Project provides practical support and financial assistance to ensure early detection of breast cancer, access to care, and treatment compliance.
Services are provided via a toll-free telephone line. An English-Spanish bilingual Care Coordinator provides Project participants with emotional support, linkages to and coordination with a variety of financial and supportive services, guidance in securing coverage for treatment, and educational materials. Project funding assists participants with transportation to breast health medical appointments, and, for diagnosed patients, home/personal health support needs, such as utility payments, lymphedema garments, head coverings, and grocery store food vouchers.
Financial assistance provided through this grant will continue until fully expended or March 31, 2009, whichever occurs first.
