CalAIM: Enhanced Care Management

Who We Are

CalAIM (California Advancing and Innovating Medi-Cal) is designed to improve the quality of life and health outcomes of Californians through a wide range of programs and policies.

Under CalAIM, the California Health Collaborative (CHC) is now offering the Enhanced Care Management (ECM) benefit program.  Enhanced Care Management (ECM) is a free benefit for eligible individuals who have Medi-Cal and are enrolled in a Managed Care Plan and does not otherwise change or lessen a member’s Medi-Cal benefits.

Enrolled members are assigned to a dedicated Lead Care Manager to help coordinate services and provide resources. Lead Care Managers will work closely with members to help them understand, navigate, and utilize the full scope of their insurance benefits. ECM is intended to help address social determinants of health and complex challenges impacting a member’s health. ECM is a person-centered program dedicated to whole person care.

Currently, CHC serves members eligible for ECM with Medi-Cal Managed Care through Anthem Blue Cross, Health Plan of San Joaquin, Mountain Valley Health Plan, Partnership Health Plan, and Central California Alliance for Health.  We provide ECM in El Dorado, Fresno, Kings, Madera, Merced, Nevada, Placer, Sacramento, San Joaquin, Stanislaus, Sutter, Tulare, Yolo, and Yuba  Counties.
