Who We Are

Thank you for your interest in tobacco prevention and education in Merced County. The Merced County Tobacco Control Program has transitioned over to the Merced County Department of Public Health (MCDPH). For more information and how to get involved, please connect with the MCDPH.

The Merced County Tobacco Control Program (MCTCP) served in the capacity of the Local Lead Agency for tobacco prevention and control efforts in Merced County from July 2017 – July 2021. With funding support from the California Department of Public Health – Tobacco Control Program, MCTCP was administered by the California Health Collaborative to implement a comprehensive tobacco control plan. Program plan strategies were developed based on results of a community needs assessment and prioritization process and adhere to priority areas and guidelines set forth by the California Tobacco Control Program.

California Health Collaborative’s integrative perinatal wellness program (s) support birthing people, moms, dads and their families at risk of a mood disorder. In addition, the perinatal wellness program (s) partner with medical providers, hospitals and behavioral health providers to create a comprehensive plan of care which supports both the patient, family, and the provider.

San Joaquin County Sista Coach Program

The Sista Coach Program is a case management program funded by San Joaquin County Public Health for Black/African American pregnant and birthing people or has an infant 0-3 months.

Each program participant works with their very own personal support advocate ( a Sista Coach) who will administer evidence-based assessments and screening tools and provide individualized support accordingly. This can include 1-on-1 meetings, going to doctor visits, and providing baby items. The participant will also receive access to a monthly support group.

ELIGIBILITY REQUIREMENTS

  • Identify as Black or African American
  • San Joaquin County resident
  • Pregnant or have an infant ages 0-3 months

WHY REFER TO A SISTA COACH?

The infant mortality rate for the African American community is currently at a rate of 11 deaths per 1,000 births and the fetal mortality rate is 14.9 deaths per 1,000 live births; more than twice the rate for white infants. (San Joaquin County 2019-2022 Community Health Needs Assessment)

Perinatal Wellness of Fresno County

Perinatal Wellness of Fresno County

Perinatal Wellness was developed to work specifically with pregnant and post- partum birthing persons experiencing mood disorders and anxiety. Our staff consists of a Care Navigator, and Maternal Behavioral Health Professional who all work together with the birthing person and her family to develop a comprehensive care plan so that she can feel better and return to wellness. Perinatal Wellness focuses on combating social determinants of health: food insecurity, relationships, employment, access to medical and behavioral health care. Through weekly one-on-one in-person visits, the care coordinator will administer evidence based assessments and screening tools, provide health education, linkage to resources, and help with appointment tracking and goal setting.

PRETERM BIRTH:

In 2019, 1 in 10 babies (10.0% of live births) was born preterm in Fresno county. The rate of preterm birth in Fresno county is highest for black infants (13.9%), followed by American Indian/Alaska Natives (12.0%), Hispanics (9.5%), Asian/Pacific Islanders (9.0%) and whites (8.6%). Our perinatal program empowers women through awareness and education in hopes of reducing this disparity.

ACE AWARE:

Our perinatal programs support both the family and provider(s) in understanding how to screen, address, and manage stress during pregnancy. Traumatic events that occur before or during a woman’s pregnancy may impact the development of her pregnancy, unborn child, and self. Our program uses a trauma informed approach that even applies to an infant in utero.

Parinatal Wellness of Madera County

Perinatal Wellness of Madera County

The perinatal wellness program was developed to work specifically with birthing persons, dads and their families to  support whole family wellness. Our staff consists of a care navigator and  maternal behavioral health professional who work together with the birthing person and their family to develop a comprehensive  plan of care. Perinatal wellness focuses on combating social determinants of health: food insecurity and basic essential items, relationships and sexual health education, employment, health and medical care, mental health and substance use/abuse.

Through weekly one-on-one in-person visits, our care coordinator will administer evidence based assessments and screening tools, provide health education, linkage to resources, and help with appointment tracking and goal setting.

PERINATAL WELLNESS: 

Perinatal wellness aims to increase access to services for moms at risk for a mood disorders by integrating:

  • Counseling services
  • Case coordination
  • Collaborative community education and awareness
  • Linkage to local support groups
  • Maternal screenings in alternative and nontraditional setting
  • Family case management and planning

Initiatives

FIRST 5 OF SAN JOAQUIN COUNTY EXPANDED HOME VISITATION INITIATIVE

Our home visitation program provides for the delivery of the Health Education curricula, which educate families on eating healthy, being physically active, and managing the household budget, to families with children ages 0 to 5. 

Children and families participating in this program receive the following services: Raising A Reader, an early literacy and family engagement program; screening and referral services for health insurance, dental care, California Work Opportunity and Responsibility to Kids (CalWORKs), CalFresh (food stamps), Women, Infants and Children (WIC) and food subsidy programs; developmental screenings through the Ages & Stages Questionnaires; and access to Parent Education Workshops.

Coalition

The Maternal Wellness Coalition (MWC) focuses its efforts on identifying service gaps through the pathways model of care.  The Maternal Wellness Coalition (s) support birthing persons, moms, dads, families, organizations, medical and behavioral health providers, who work with perinatal families at risk of perinatal depression.  The MWC partners with elected officials, law enforcement, medical facilities, school districts, and community organizations to collaborate on various medical, mental wellness, as well as social determinants of health which impacts individuals, children and families. The MWC has stream-lined their resources for expecting moms, dads, and families to receive the best outcomes. Our monthly coalitions are currently accessible in  Fresno, Madera, and  Kings County, as well as a statewide collaborative.

The Maternal Wellness Coalition (MWC) consist of a group of community programs and organizations in each county that volunteer to collaborate on perinatal health issues, particularly perinatal mood disorders and anxiety. The MWC works to assist women and men seeking community services by creating a collaborative system of care.
